All application materials must be submitted online by Friday, March 16th at 5:00PM. Late applications will not be considered. ONCE YOU START THE FORM, YOU ARE NOT ABLE TO SAVE AND GO BACK TO IT. PLEASE FILL OUT THE ENTIRE FORM BEFORE YOU CHECK SUBMIT AND SAVE. You must check submit AND press save for the application to be sent. Only complete applications will be considered.
Applications are to be filled out by adults who are part of an empowering youth-adult relationship, i.e. mentors, after school program leaders, youth volunteers, teachers etc. Only adults/youth who are involved with structured Massachusetts-based youth development or mentoring programs are eligible
The nominating adult must have had a relationship through structured programming with the young person for at least one year. Relationships under 1 year WILL NOT BE CONSIDERED.
Applications must include one supplemental letter of recommendation for the young person, not from the nominating adult and from a non-family member, i.e. program director, guidance counselor, etc. The recommendation letter cannot be written by the person submitting the application. (UPLOAD THE RECOMMENDATION LETTER IN THE UPLOAD SECTION BELOW) The stories that you tell throughout the application allow reviewers to better understand the young person, giving him/her a more competitive application. The more in-depth the stories, the better.
You must upload one picture of you and the young person. (UPLOAD THE PICTURE IN THE UPLOAD SECTION BELOW)
The award must be used for the young person's educational pursuits, including but not limited to: college or other post-secondary education; private school tuition; summer scholastic programs; tutoring (including SAT and MCAS); supplemental classes in specific areas of interest, such as art or music; school supplies, books, etc.
There is no age requirement for the young person.
Applications will be evaluated according to the following criteria:
- Demonstrated perseverance through adversity
- Strength of mentoring relationship
- Evidence of young person striving for academic development or advancement
- Specific plans for use of award money
- Demonstrated need for award money
If awarded, MMP requires the following:
- Attendance by young person and adult at the award presentation at the Champions of Mentoring breakfast on Wednesday, June 6, 2018 at Fenway Park
- Signed permission form from the young person's parent or guardian
- Regular reporting on use of award money
